PSA Copy of Birth Certificate
Certified True Copy of Recent Report Card (First to Latest Grading Available)
Certificate of Good Moral Character
ESC Certificate Once Accepted (for G8-G10 ESC Grantees)

For Foreign Students

Two 2"x2" colored ID pictures
Original and Photocopy of Passport
Original and Photocopy of Parents' Passports
Original and Photocopy of Visa or ACR I-card
Notarized School Records in English translation
Special Study Permit (once accepted)
